This website, (the "Site"), is provided to you by Pima Lung & Sleep, PC ("PLS," "we," "our," "us").  We respect the privacy of your information and are committed to protecting the personal information that you ("you," "user") may disclose through the use of our Site.  This Privacy Policy describes how we will use and protect your personal information.  This Privacy Policy is a part of our Terms of Use, which you are required to accept in order to use the Site. 

By using this site, you agree to be bound by the terms of this Privacy Policy.  If you do not consent to the collection and use of your information as set forth below, you should not use this Site!

Changes to Our Policy

We reserve the right to modify or amend this Privacy Policy at any time. All changes to this Privacy Policy will be effective immediately upon their posting to the Site. Material changes will be posted conspicuously on this Site for a period of time. Information collected before changes are made will be treated in accordance with the previous Privacy Policy Each version of our Privacy Policy will be prominently marked with an effective date.

Continued use of the Site after the effective date of a Privacy Policy will indicate your agreement to any modified terms.

Our Collection and Use of Your Non-Personal Information

We collect anonymous information from visitors to the Site. Such information can not be used to identify a particular person ("Non-Personal Information").  Some examples of Non-Personal Information include IP address, browser type, pages viewed, city, and state.

We use Non-Personal Information in the aggregate to determine how much traffic the Site receives, to statistically analyze Site usage, and to customize the Site's content, layout, and services. In addition, we may use your IP address to help diagnose problems with our server, to manage the Site and to enhance the Site based on the usage pattern data we receive. Forms Submitted to Us Via Email

When you email filled out forms to us, such as the Patient Intake Packet, you may be transmitting your Personal Information (e.g., name, address ...etc.) to us. When emailing us these forms, you will not alter the attribution of origin of the electronic mail. The email must identify the sender accurately.

We collect emailed forms and use the corresponding information to help provide you with our medical services.

Handling of Electronic Records

In general, we will retain all information collected through the Site unless otherwise prohibited by law. We maintain backup files as a protection against natural disasters, equipment failures or other disruptions. Removing a record from our main files does not remove that record from any backup systems. Additionally, information deleted from our main files for any reason is not actively deleted from our backup systems. Such data will eventually be passively deleted as backup records are erased through the normal recycling of backup files. In the meantime, as long as backup records exist, they receive the same security protections as our other records.


We employ technologically reasonable and current methods to help prevent unauthorized access to our website database and to maintain data accuracy and integrity. Although we have such precautions in place, no data transmission over the Internet or any network can be guaranteed to be perfectly secured. As a result, while we try to protect your information, we cannot ensure or guarantee the security of any information that you transmit to us, and you do so your own risk.

Third Party Practices

This Privacy Policy addresses only the use and disclosure of information we collect from you. If you disclose information to third party websites, even those linked from our Site, different rules may apply to the third party's use and disclosure of the information disclosed to them. We do not control the privacy policies of third parties, and you are subject to the privacy policies of those third parties where applicable. We strongly encourage you to review the privacy policy and any other policies of third party sites before disclosing your personal information or engaging the third party for services.

Children Under the Age of 13

We respect and are committed to protecting the privacy of children. No part of the Site, including the Site itself or any services made available through the Site, are designed or intended to attract children under the age of 13. No personal information is knowingly collected from a child under the age of 13 via this Site.
