Referring a Patient is EASY as 1, 2, 3!
- Send us the patient's information.
- We will contact the patient for you.
- You will get a full consultation report.
- Phone or fax.
- Standard mail or e-mail to patientforms@pimalung.com.
- Fill out and submit electronically our NEW online Procedure Scheduling Form
- Download and print forms from our website.
- Submit referrals through Medical Referral Source. Go to medicalreferralsource.com and submit electronically.

You can rely on our follow-up too.
Once you send us the procedure scheduling form, we'll make every reasonable effort to ensure the patient follows through and seeks the prescribed treatment. If we can't reach the patient, we'll send a letter notifying the referring physician that the patient has not responded to our attempts at contact.After the patient has completed the tests, we'll send a complete follow-up to the referring physician's office. Our dictation will be faxed within 24 hours of completion of tests.
